- Fax – Fax your completed registration form, including full payment by credit card, to +1-608-395-2001. In order to qualify for the early bird rate, faxed registrations need to be received no later than 11:59 p.m. CST on May 20, 2010.
- Mail – If you plan to pay by check or wire transfer, please send your completed registration form

Mailed registrations must be postmarked on or before May 20, 2010, to qualify for the early bird rate.

Cancellation Policy
If you register before May 20, 2010 and do not receive a visa, your registration will be refunded, minus a US$100 processing fee. If you register after May 20, 2010, do not receive a visa and cancel by June 11, 2010, your registration will be refunded minus a US$200 processing fee. If you register after May 20, 2010, do not receive a visa and cancel after June 11, 2010, no refunded will be provided.
